Many residents had questions and concerns after the recent presentation on “Advance Directives and Palliative Care”.
To provide additional information on other advance directives and assist in filling out the Health Care Proxy (HCP) form, Eileen Egan, RN, Esq., and Mary Kovoor from Phelps Hospital will be in the Gathering Room on November 8, 2 pm to 4 pm for a walk-in session.
HCP forms will be provided if needed. Forms are also available in the Resident Care Center (RCC), if you wish to get one before the session.
